I've never seen that happen. If the sheet of LDPE was scratched prior to cutting the wad, It will be weakened at the scratch. Maybe enough to end up with what you show. Don't know. That's how I cut my strips of LDPE prior to punching wads. I score the sheet and pull off the strips.

I've picked up a lot of wads but never seen one like that. All I've ever picked up could have been reused. No idea how that would happen unless it hit something after exit ??
I have some plastic sheeting I've made wads from but prefer the .060 vegetable wads I punch out from Diesel head gasket material.
In the percussion revolvers I've been using the Duro-felt wads made of the hard .125 wool felt that seem to be working very well. I get tired of smearing grease over the ball ends in the chamber mouth with these guns so instead punch the felt wads out and soak in bullet lube and seat them under the ball.
Gerald - my guess is you are seating the wads with the expansion die or the bullet ... using the powder compression die, I have never had the LDPE issues you posted
Never found one of mine that look'd like that.
Most of the time. The ones I have found look like they could be used again.
How much, and just HOW are you compressing the wad and powder?

Every instance of case separation I have ever seen involved the use of LDPE wads. Your rifle is telling you it doesn't like that particular set up. I would go to fiber wads and never give it a second thought. Shoot straight, rdnck.

First time I had separation was yesterday. No poly wads but.
New 45-90 Starline brass. Paper patched a Baco slick and thought I would breech seat and let the case fireform.
Filled the case with Wano 3F for a change as I had already done the same with 2F OK.
First shot hit the target. The next two missed and bust the cases an inch back from the mouth so I gave it away.
First time firing the cases and the only wad was a thin card at the mouth.
Got me bluffed as to the cause.
